This information covers the group website. Patient enquiries and medical records are handled by the relevant healthcare provider. Use the clinic’s contact channels for healthcare enquiries. Do not include personal identity numbers, medical records or other sensitive information in the group contact form or ordinary email.

When you contact us

The contact form contains your name, email address, phone number and message. These details are used to receive, answer and follow up your enquiry. Processing is based on our legitimate interest in handling incoming enquiries. Where your enquiry concerns a contract, processing may also be necessary to prepare or perform that contract.
